Custom Tattoo Request Form

This form is for requesting a custom tattoo. To avoid any confusion, please read my FAQ page carefully before submitting a request form.

If you are looking to book an appointment for a FLASH tattoo or one of my PRE-DRAWN designs, you can submit those requests here.

I require a deposit of $100 to secure an appointment time and compensate for my time spent designing the tattoo. This deposit goes towards the final cost of your tattoo. This deposit is NON-Refundable, even in the event of an emergency. I accept deposits via Venmo, Cashapp, and Paypal, or you can pay in person with Cash. You can find my payment information here.

Please be as specific as possible, for example “right upper forearm” instead of “arm”. If you don’t know where you want this piece, you can say that!

**Important Note: I will NEVER copy, rework, or tweak another artist’s tattoo design. I will also never design s tattoo based on any AI-generated images. Any requests to do so will be denied.**

Please choose which days of the week you are typically available.

Please refer to the following guide to factor in your budget. You do not have to specify which tier your tattoo falls under, just give me a ballpark dollar amount!
